Our first session together will focus on getting to know one another and identifying your goals for therapy. We will begin to develop a plan of how you would like to reach your goals, how often we should meet, and discuss if you would like anyone else to be a part of this process. Here at Positive Mind Counseling, our goal is to offer a supportive, non-judgmental, and safe space where we can collaborate and build a strong therapeutic relationship that helps you reflect, heal, and grow.

If you are unable to complete the forms ahead of time, no problem, we will also have copies available to you in our waiting room that you can complete 15 minutes prior to your scheduled appointment.
